Here is a description of your first visit to our orthodontist.
During your initial consultation at our office, our orthodontist will thoroughly discuss with you the various treatments that we offer including those that specifically suit your needs. We plan to take x-rays, capture photos, and complete a full exam in order to determine the best course of action for your oral health. We will guide you through and will be ready to answer any questions that you may have about your oral health in general.
New Patient Documentation
Before the appointment, you will be required to fill out new patient forms along with providing any prior dental history. We suggest that you arrive at least 20 minutes prior to your scheduled appointment to ensure that you have plenty of time to fill out these forms:
For your convenience, our office also provides the choice of filling out the forms before coming to the clinic. This means that you may fill out the enrollment forms online. This saves you time and ensures an efficient and effective visit. Patients can download the health history forms above and fill in all the required details. This information will be stored in our database and evaluated prior to your session.
